Fox Army Medical Home

The Patient Centered Medical Home is a patient-centered system of health where patients are involved in their care with a team of health care professionals that knows your medical needs, anticipates periodic routine screenings, and coordinates with other health care delivery teams to provide you the best quality care. The key to healthy outcomes for our beneficiaries is establishing and sustaining a continuous relationship of sharing medical information between the patient and the healthcare team (providers, nurses, and administrative support personnel). The Army Surgeon General stated Army Medical Homes provide access to a system of health demonstrating commitment to Soldiers, their families, and retirees that brings value and inspires trust in Army Medicine.

How Does Fox AMH Work?

Every beneficiary assigned to the Fox Army Medical Home will have a primary care manager (PCM) designated by name and a designated support team. The comprehensive health care team includes behavioral health, pharmacy, dieticians, and case managers to assist you and your PCM to achieve optimal health through a comprehensive care approach in a primary care setting.
 
  • Fox AMH encourage our beneficiaries to enroll in the MHS GENESIS Patient Portal, a secured messaging system with your healthcare team to address general health questions not requiring an office visit.
  • MHS GENESIS also allows the healthcare team to communicate with beneficiaries when a change occurs such as reassignment of Primary Care Provider due to retirement.

Patients participate in their care by partnering with the health care team to develop comprehensive care plan that includes:
  • preventive health measures.
  • chronic disease management.
  • a focus on health behaviors.
  • health maintenance.

Patients can monitor referrals through www.humana.military.com
 
Patients have 24/7 access to care through either an assigned primary care team during normal operating hours or an after-hours nurse advice line (800-TRICARE, option #1).

Patients also are able to use the Nurse Clinic on Tuesdays and Thursdays from 9 to 11 a.m. for the following: upper respiratory infection, urinary tract infection, pregnancy testing, contraceptives.

  • We know things may occasionally happen that will cause you to cancel your appointment. Please call us whenever you need to cancel (24 hours in advance when possible) so that someone else may use the appointment slot.
  • Showing up for your appointments on time allows your team to provide you the best care and the best care for all our patients.
  • If you see a civilian provider, please remember to bring those test or treatment results to your next FAHC appointment.
