OPEN ENROLLMENT AT FOX IS HERE!
Fox enrollment for TRICARE Prime beneficiaries is now available. If you're a TRICARE Prime beneficiary and would like to change your primary care provider to Fox Army Health Center, call Humana now at 800-444-5445.
Once you have called Humana to change your provider, you can make an appointment at 256-955-8888 ext. 2.
Please note: if you are aged 62-64, you are encouraged to remain with your current health care provider as you will need to transition out of FAHC again at age 65.
