The laboratory closes at noon on 1st and 3rd Fridays.
The Call Center closes at 3 p.m. on  3rd Fridays for training.  
Fox Army Health Center will close at noon on Friday, August 15, 2025 for staff town hall.

Active Duty Readiness

Periodic Health Assessments

1: Service Member (SM) must first complete Part 1 (member portion) online at https://www.Mods.army.mil/ Medical Readiness Portal. The PHA survey is to be completed before your face-to-face appointment.  
2. Call the appointment line at 256-955-8888, option 4. You will be scheduled for both Part 1 records review with the nursing staff as well as your Part 2 with the provider.
3. For your records review appointment, please fast for at least 8-12 hours, you may drink water. Bring your eyeglasses if you wear prescription eyewear.
4. On the day of your records review appointment, check into the Primary Care Department on the first floor.
5. You will be given a checklist by the front desk to complete labs, vision screening, hearing and dental as applicable.
6. Once the checklist has been completed, return to Primary Care to complete the Part 1 visit.
7. On the day of your appointment for Part 2 face-to-face, you will check into the Primary Care Department.

**If you part 2 is a virtual appointment, you will be called or be sent a My Military Health Virtual Visit telehealth link to check-in/start your virtual appointment. **

Specialty Military Physicals

Appointments for specialty physical examinations required for flight operations, military school attendance, other unique military programs, commissioning and retirement physicals are also a two-part physical.  
DD Form 2807 and DD Form 2808 (forms below) will need to be filled out and brought with to your Part 1 records review appointment.
1. Call the appointment line appointment line at 256-955-8888, option 4. You will be scheduled for both Part 1 records review with the nursing staff as well as your Part 2 with the provider.
2. For your records review appointment, please fast for at least 8-12 hours, you may drink water. Bring your eyeglasses if you wear prescription eyewear.
3. On the day of your records review appointment, check into the Primary Care Department on the first floor.
4. You will be given a checklist by the front desk to complete labs, vision screening, hearing, dental, x-ray, an electrocardiogram as applicable.
5. Once the checklist has been completed, return to Primary Care to complete the Part 1 visit.
6. On day of your Part 2 face-to-face appointment, you will check into the Primary Care.
*Reserve and National Guard Soldiers must have active-duty orders to receive a physical examination, or Periodic Health Assessment
